Example Case Study 1: Preparation and recovery from surgery Osteoarthritis is a leading contributor to global mobility impairment
Example Case Study 1: Preparation and recovery from surgery Osteoarthritis is a leading contributor to global mobility impairment, driving the rapidly increasing demand for total knee replacement (TKR) surgery, up 23% since 2009 (Metcalfe et al., 2015). Over 100,000 TKRs were performed in the UK annually (National Joint Registry, 2018), resulting in substantial healthcare costs (average £11,400 per surgery) (NHS, 2019). Health promotion interventions, such as pre-surgical education (“prehab”), are vital in reducing patient pre-operative anxiety, optimizing surgical outcomes, and reducing hospitalization costs. Specifically, lower health literacy is the single best predictor of poor health outcomes (Perugini, 2010; Weiss, 2003): it is associated with increased anxiety (Edwards et al., 2017; Ruffinengo et al., 2009; Sjöling et al., 2003; Spalding, 2003), poorer disease management (von Wagner et al., 2009; Wolf et al., 2009), more complications after surgery (De Oliveira et al., 2015), negative functional outcomes (e.g. reduced function, slower recovery) and increased costs (Canadian Institute for Health Research, 2019). Furthermore, anxious patients are less likely to follow post-operative precautions, engage in more risk behaviours (., inactivity) and experience more painful and complicated recovery (Upton et al., 2012). Pre-surgical education (prehab) improves patients’ post-operative outcomes and reduces hospitalization costs through several pathways, including both increasing patients’ health literacy (De Oliveira et al., 2015) and reducing pre-operative anxiety. As a researcher who works with patients, you are asked to suggest evidence-based strategies to find the reasons why patients avoid preparation for surgery and recommend solutions for increasing patients’ preparation for their surgery.
