In the 1st column mention the do not mention the consequences, mention the hazard alone.
In the second column mention how the worker will be harmed. mention specific injuries
Do not mention no further action in 4th column.
Please complete all columns adequately as you have done a poor job of mentioning most hazards with proper information.
Part 3:
Mention general legal according to ILO C155
Change the 3 actions to technical controls do not mention admin or PPE controls.
Explain the severity with specific injuries
All sections need technical explanations, refer online sources if you need.
Part 4 provide more explanation for communication and follow up section.
Your report needs to be improved by a great deal, refer the sample anad guidance documents and follow similar method, do not copy from them.
