Psychosocial History Report Guidelines
In writing your Psychosocial History:
Limit to 3 double-spaced typed pages.
Follow the outline provided (fictional history).
Write in full sentences and paragraph.
Use no real names.
Report only the information taken from the parent(s), not that from the child or
teacher, a school record, or your observations.
Do not include mention of yourself, e.g. this social worker.
Write in either the present or past tense consistently.
Avoid use of contractions, e.g. do not, rather than don’t; will not, rather than
won’t.
Do not use slang or informal words, e.g. kid, etc.
Use formal titles, e.g. Mr. or Mrs., not Sue and Don, grandmother and
grandfather, not grandma and grandpa.
Use quotes very sparingly. A few words here and there – descriiptive,
rather than factual.
Do not overuse noting information from the parent, e.g. Mrs. Jones states,
reports, etc., but always state sensitive information as
coming from the parent.
When noting ages or grades, use numbers or words consistently, e.g.
2 years or two years, grade 4 or grade four. Note years after ages,
e.g. 4 years rather than 4.
