Review the Case Study: You are the nurse caring for John Anderson, 68 years old, and was admitted to the medical unit with exacerbation of COPD and bilateral bacterial pneumonia. Vital signs upon admission to the unit are–Blood Pressure 156/88, Pulse 101; Respiratory Rate 28, Temperature 102.4°F. The client reports worsening shortness of breath, fatigue, decreased appetite, and a weight loss of 15 pounds over the past three months. The nurse notices the client is lying flat in bed and currently on 2L O2 via nasal cannula with an oxygen saturation level of 88%. Other symptoms include a dry cough, inspiratory and expiratory wheezes on auscultation of breath sounds. The client arouses easily but often falls back to sleep. The client appears restless and is often confused by the nurse’s questions. The client is oriented to person only. Respiratory treatments (albuterol) are ordered every 4 hr as needed. The client is deaf and has a past medical history of a left leg below the knee amputation from a war wound. The client reports intermittent homelessness and heavy alcohol use.
